Instructor-Agency
Orientation Questionnaire -- Optional -- This questionnaire
has been designed to help instructors to orient themselves to agencies
or organizations that they are considering using as community partners
for service-learning courses. The information obtained should (i) clarify
the degree of "fit" between an organization and a particular
course; (ii) help determine factors (e.g., number of students, community
members' needs) that might constrain projects included in a course syllabus;
and (iii) form the basis for an orientation program that could be provided
to students planning to serve at the site.
Instructor-Agency
Agreement -- Required of Instructors-- This two-part form
must be signed whenever an instructor plans to send students enrolled
in a service-learning course to a specific agency or organization. The
form asks both the instructor and the agency to spell out the expectations
they bring to the partnership and any restrictions that they will place
upon the students involved in it. The goal of requiring the form is
to reduce the risk of serious misunderstandings once the placement is
made.
Student
Self-Orientation Questionnaire -- May be required by the Instructor
-- This questionnaire has been designed to help students to orient
themselves to agencies or organizations at which they are considering
serving as part of a service-learning course. Its use is particularly
appropriate if neither the organization nor the instructor is able to
provide a purpose-designed orientation program.
Student-Agency
Agreement -- Required of Students -- Each student in a service-learning
course must complete a Student Service-Learning Agreement before
beginning work at a community partner site. The student, the on-site
supervisor (or another representative of the community partner), and
the course instructor must sign the Agreement, and each party should
retain a copy of it. Because different courses and different community
partner arrangements will require that specific issues be addressed
in specific ways, the College does not offer a standard form for this
purpose. However, it does mandate that a number of topics be explicitly
addressed in every Student Service-Learning Agreement.
Liability
Waiver -- Required of Students -- Each student in a service-learning
course must complete a Student Service-Learning Liability Waiver
before beginning work at a community partner site.
Service-Learning
Log Sheet -- Required of Students -- All students must systematically
log the hours that they spend in service activities. This form provides
a basic template for such record-keeping.
