Student Course Evaluation Form

This form is intended to help me "steer the ship" as we navigate the semester. Please fill out this form on the weeks it is suggested (after the midterm and a few weeks before the final). This counts for class participation but you need not attach your name to the evaluation if this will make you uncomfortable. The form is intended to let me know what is working and what is not working so it is helpful to hear both praise and criticism concerning the class.

Thank you for taking time to fill out this form. The more information you give me the better I can adjust the course. You will also be asked to fill out an official school evaluation at the end of the semester. Since I don't get those back until after the semester ends they are useless for adjusting current courses. Please fill out as much as you want on this form and then submit it to the professor. It will be sent to him in the form of an E-mail message.
